The following health information is available at the student's health portal:

• Summary of clinical encounters at Health Services
• Lab results
• Immunizations administered at Health Services
• Medications

TRANSFER / RELEASE OF MEDICAL RECORDS

Medical records can be released with written authorization from the patient. If patient is less than 18 years of age, a written authorization from a parent or legal guardian is required. A Medical Release Authorization form must be filled out in person at the Health Services. If that is not possible, the patient may fax a notarized copy of the completed form to (212) 217-4191.
